Member Agency Food Order Form

Second Harvest of Greater St. Joseph is pleased to provide this online order form for you to place your order for food items. Please carefully note the following prior to using the form.

Once you have completed the form, print your browser window with the completed form prior to submitting it. This is a simple form program and you will not receive an itemized receipt or confirmation. If there is a problem during submission, your print-out will be your only record of the order.
Immediately following submission, you will receive a screen that tells you that our web server has received your online order form. However, it is NOT a confirmation that your order has been received by Second Harvest. Please watch your email (the one you listed on your online order) for a Reply by Second Harvest, when your order has been received at the Food Bank.
You should receive an email confirmation within 24 hours on all orders received at Second Harvest from Monday, 8:00 am - Friday, Noon.
This form will allow you to place an order for 40 items. If you need additional items, please submit this form and complete and submit a second form.
